Accident investigation report: 25 people including the Party Secretary of Qingxi Town were held accountable, and a fire in a factory building in Dongguan resulted in 7 deaths and 5 injuries

The website of Dongguan Emergency Management Bureau released the "Investigation Report on the" 9.2 "Major Fire Accident in Qingxi Town, Dongguan City" on September 12.

On September 2, 2022, at around 11 o'clock, a major fire accident occurred in an old idle factory located in Hongmenshan Industrial Zone, Changshantou Village, Qingxi Town, Dongguan City. The area of the fire was about 14600 square meters, resulting in 7 deaths and 5 injuries. Some building structures, machinery and equipment, and items were burned, resulting in a direct economic loss of about 10.4839 million yuan.

Recently, the Dongguan Municipal Government approved the investigation report on the "9.2" major fire accident in Qingxi Town, Dongguan City. After investigation by the accident investigation team, it was determined that the "9.2" major fire accident in Qingxi Town, Dongguan City was a production safety responsibility accident caused by the failure of the enterprise to fulfill its main responsibility for fire safety and illegal hot work operations.

The accident investigation team found out that the direct cause of the accident was that Dongguan Guishun Renewable Resources Recycling Co., Ltd. employed personnel without special operation permits, and used an injection cutting torch in violation of regulations to cut the metal pipe hanger in the middle area of the second floor of the workshop A of Dongguan Yuanzishan Edible Fungi Co., Ltd. When the cutting torch flame ignited polyurethane foam insulation materials, causing flashover and spread to disaster.

The investigation found that the involved companies Guishun Company and Yuanzaishan Company have not implemented the corporate fire safety responsibility system, weak risk awareness, lack of safety education, and superficial safety management; The owner unit, Changshantou Stock Economic Cooperative in Qingxi Town, Dongguan City, has problems such as inadequate performance of regular safety inspection responsibilities; Dongguan Guochuang Property Management Co., Ltd. has issues such as failure to coordinate and manage the construction party uniformly; The actual contractor involved in the dismantling of insulation materials and wall repair project has problems with on-site fire safety management, lack of education and training, etc; Guangdong Xinghe Biotechnology Co., Ltd. has issues with not obtaining fire safety acceptance and filing after changing the use of the factory building; Dongguan Yushi Decoration Co., Ltd. has issues with illegally lending company qualifications and business licenses.

There are certain problems in fulfilling the safety supervision responsibilities of the Qingxi Town Committee and Government, the Municipal Fire Rescue Detachment, the Municipal Agriculture and Rural Bureau, the Municipal Bureau of Commerce, the Municipal Comprehensive Law Enforcement Bureau of Urban Management, the Municipal Emergency Management Bureau, the Qingxi Fire Brigade, the Qingxi Town Agriculture, Forestry and Water Bureau, the Qingxi Town Economic Development Bureau, the Qingxi Town Urban Management Branch, the Qingxi Town Emergency Branch, and the Changshantou Village Committee of Qingxi Town in Dongguan City.

The public security organs have lawfully investigated four individuals, including Li Yigui, the legal representative of Guishun Company, Lai Yuanyuan, the legal representative of Yuanzai Mountain Company, Chen Tangjun, the administrative manager, and Tan Xiaowei, the security team leader, and transferred them to judicial organs for criminal responsibility.

The relevant departments dealt with Guishun Company, Yuanzaishan Company, Changshantou Economic Cooperative in Qingxi Town, Dongguan City, Dongguan Guochuang Property Management Co., Ltd., Dongguan Yushi Decoration Co., Ltd., Guangdong Xinghe Biotechnology Co., Ltd. and Qingxi Branch, as well as Deng Baoming, the actual contractor of the insulation material demolition and wall repair project, and two other relevant enterprise personnel in accordance with the law; Include Guishun Company, Yuanzaishan Company, and Changshantou Economic Cooperative in Qingxi Town, Dongguan City as joint punishment targets. For personnel involved in the enterprise, they shall be handed over to relevant units for handling in accordance with the party organization relationship and regulations.

The disciplinary inspection and supervision organs have held a total of 25 public officials from the party committee, government, and relevant functional departments in the accident area accountable. Among them, Ye Jinrui, the Secretary of the Party Committee of Qingxi Town, Dongguan City, was given serious criticism and ordered to conduct a written inspection; Shen Zhipan, Deputy Secretary of the Party Committee and Mayor of Qingxi Town, Dongguan City, has been given a government warning.
