At the bimonthly consultation meeting of the Xuhui District Political Consultative Conference on "Taking Medical Reform as the Leadership, Optimizing and Improving the Community Health Service System, and Strengthening the Health Network of Residents" held recently, experts such as Wu Jinglei, President of the Shanghai Medical Association, and Wu Qianyu, First level Inspector of the Shanghai Health Commission, shared their insightful insights on public health talent cultivation, mechanism optimization, and supply enhancement.

Luo Huapin, deputy head of Xuhui District, introduced that although Xuhui, as the central urban area of Shanghai center, has many top three hospitals, community medical resources were scarce at the beginning. For this reason, since last year, Xuhui District has proposed to build a "life box" around neighborhoods, party and mass service positions, and to layout the "four piece set" of community health service stations, community canteens, community bathing aids, and community cultural and sports facilities in the "15 minute community living circle", and improve facility planning and layout.

It is reported that within this year, Xuhui District will complete the renovation and expansion of community health service centers in streets such as Xujiahui and Kangjian, and simultaneously complete the construction of the new campus of Lingyun Community Health Service Center located at 1139 Luoxiu Road. The renovation and expansion projects of community health service centers in streets such as Tianlin and Fenglin will also be launched within the year. In combination with the construction of the "life box", Xuhui District will complete the optimization and upgrading of more than 11 community health service stations this year, involving streets and towns such as Huajing, Fenglin, Kangjian, Xujiahui, Caohejing, Longhua, Changqiao, and Hunan, striving to achieve a compliance rate of over 80% for the entire district's health service stations by 2025.

While consolidating community health service centers and health service stations, the provision of medical facilities for public cultural and sports facilities has also attracted attention. Overall, Shanghai has been promoting the deployment of AED and other equipment, with some layout in public places such as airports, train stations, and shopping malls, but sports venues do need further improvement." He also mentioned that training for sports venue personnel urgently needs to be improved, including general first aid training, defibrillation training, and corresponding recognition abilities.

In addition, emergency equipment such as AED often faces the need for replacement due to its high cost, especially after reaching its service life. Therefore, various sectors of society can explore the introduction of new mechanisms, such as only replacing batteries for applicable equipment, improving the efficiency of emergency equipment use, reducing layout costs, and seeking broader social consensus under reasonable and compliant conditions.

The latest statistical data in Xuhui District shows that there are currently 13 community health service centers and 64 community health service stations among the 1.1147 million permanent residents in the district. Among them, as of 2022, the community health service centers have actually opened 1114 beds, with the number of beds per thousand people reaching 1.0, which is higher than the city's average level. In terms of personnel allocation, as of the end of last year, there were a total of 1896 health technicians and 771 practicing physicians in the community health service centers in the entire region, including 553 general practitioners and 786 nurses. When it comes to talent cultivation, Wu Qianyu mentioned a phenomenon: in streets such as Tianping and Hunan, although the location advantage is good, not all talents are interested in community health service work here, one of the reasons is that "the platform is not large enough". The common challenge faced by public health services is that there is no shortage of talents, but excellent talents are needed. Wu Qianyu suggested that if the staffing of community health service centers is insufficient, they can be optimized through positions such as doctor assistants to assist family doctors in completing basic health management work, such as daily phone calls to residents.

Wu Jinglei mentioned thoughts on the income of doctors in community health service centers. The community health service center in Xuhui ranks among the top in terms of per capita resources, government investment, service personnel, and service level in the city. With the increasing work content and pressure of community doctors, Xuhui may explore more reasonable and optimized salary mechanisms to keep the income level of community hospital doctors in line with the development process of the economy and society.

Embrace smart healthcare

On the eve of the 2023 World Artificial Intelligence Conference, the application of smart healthcare and artificial intelligence technology in the medical field is also a focus of attention for attending experts. Mo Xiaofen, Assistant Dean of the Eye, Ear, Nose and Throat Hospital Affiliated to Fudan University and Director of the Double First Class Construction Office, suggests that in the process of gradually improving the community medical service center system, investment in the construction of smart community health service centers can be increased, and smart medical technology can help community medical service capabilities achieve rapid homogenization.

She said that using mature AI technology can be applied in fields such as remote diagnosis, intelligent image recognition, and ophthalmology. For example, loading intelligent diagnostic devices on commonly used lights by ophthalmologists is equivalent to adding a digital team to community hospitals. Similar devices can also load the naked eye 3D function. If the community hospital is difficult to diagnose and treat difficult and complicated diseases, real-time consultation can be achieved through remote 5G. Doctors in Grade III A hospitals can guide the community health service center without leaving home, which is equivalent to providing a "invisible" team of experts for online diagnosis and treatment in the community, helping the community hospital quickly improve diagnosis and treatment capacity and efficiency.

Experts attending the meeting also mentioned that Xuhui may take the lead in forming a synergy of data on specialized disease management in the entire region. At present, six hospitals in the city have had relatively perfect cases. The diabetes data of the hospital is judged by integrating daily management data, and analyzed and integrated with modern AI technology.
