Zhao Bin, the director of Jing'an District and the Elderly Care Home, was once troubled by one thing - the medication and medical treatment of elderly residents. There are over 160 elderly people living in the annual welfare home for the elderly, with an average age of 83 years old. Each elderly person has an average of three or more chronic diseases, and taking medication and seeking medical treatment in the hospital are common occurrences. In the past, the daily medication for the elderly was registered and purchased by their family members at hospitals outside, and then regularly sent to nursing homes; Once the elderly need to be admitted for treatment, the nursing home should notify their family members to come to the hospital in a timely manner and take them to the hospital for treatment. "Sometimes family members may not be able to deliver medication in a timely manner due to emergencies, or when an elderly person falls ill, they may not be able to contact them for a while. We are really anxious!" Zhao Bin said. The elderly's family members are even more anxious: "They are most afraid of receiving calls from nursing homes. As soon as a call comes, I know that there must be something wrong with the elderly's health. Let's put down work and rush over as soon as possible, afraid of delaying the elderly's condition!"

The vast majority of elderly care institutions in Shanghai are facing the problem of being able to meet the needs of the elderly due to insufficient medical resources. Jing'an District is trying to solve this problem.

Today, the reporter learned from a media communication meeting held by the Civil Affairs Bureau of Jing'an District that Jing'an District has pioneered the "Five Bed Linkage" home and community care service model, which establishes service referral, information exchange, and resource sharing between the "Five Beds" of family nursing beds, nursing institution beds, family hospital beds, medical beds, and hospice care beds, allowing the elderly to truly achieve "medical care for the elderly" and "care for the elderly" in home and community care.

Build a "green channel" from nursing homes to community hospitals and designated hospitals

Grandpa Su, who is 96 years old, is a resident elderly person living in Jing'an District and the Elderly Care Home. One day, the elderly suddenly experienced symptoms such as mental fatigue and refusal to eat or drink water. The welfare home immediately notified the contracted doctors at the community hospital. After the doctor came to visit, he treated the elderly and planned to undergo catheterization for diagnosis and treatment; However, due to the elderly suffering from other diseases, catheterization failed. Community doctors immediately changed the plan and referred the elderly to the higher-level hospital - Shibei Central Hospital for treatment through the "green channel" as soon as possible. The old man quickly turned from danger to safety. After the elderly person's condition stabilizes, they are transferred to the elderly care bed at the community hospital for further treatment; After the elderly have basically recovered, they will be transferred back to the nursing home and continue to rest.

After nearly 20 days of treatment and care, the elderly person has finally recovered from being bedridden and requiring comprehensive care to be able to take care of themselves in daily life. "The 'Five Bed Linkage' mode is very good. Throughout the process, according to the patient's condition, where the elderly should go for treatment and care is reasonably settled. Our family members are worry free and at ease," said Grandpa Su's family.

A survey shows that the most troublesome thing for elderly people and their families is to seek medical treatment when they live at home or in the community for elderly care. Shen Lianxin, Deputy Director of the Civil Affairs Bureau of Jing'an District, said that the "Five Bed Linkage" model aims to solve the difficulties faced by elderly people living at home and in the community in seeking medical treatment, and truly connect the channels between home, community elderly care, and medical institutions.

When the living environment of the elderly changes, elderly care institutions set up green channels and prioritize the arrangement of family nursing beds or nursing institution beds. When the physical health status of the elderly changes, elderly care institutions timely connect with member units of the "Five Bed Linkage" agreement, such as community health service centers or regional medical centers, complete the circulation of health records, and open corresponding green channels to establish family beds or transfer to medical beds. When the health status of the elderly improves, they can be transferred back to family nursing beds or nursing institution beds through green channels. When the elderly are dying, priority is given to staying in hospice nursing beds and providing end-of-life comfort services. The entire process can achieve hospitalization, rehabilitation care, and stability for the elderly during treatment. "The integrated health and elderly care services of daily life care and end-of-life care solve the worries of elderly people's home-based and community elderly care," said Shen Lianxin.

Medications are automatically delivered to the bedside, so family members no longer need to go to the hospital to prescribe medication

The "Five Bed Linkage" mode not only plays a role in emergency situations such as the admission and treatment of the elderly, but is equally important in their daily lives.

How to reduce restlessness, may sleep on "five beds"... From this bed to that bed, an elderly family | five beds | elderly person

"The medicine you ordered is currently being delivered and is expected to be delivered before 10 o'clock." At 8 a.m. that day, the Elderly Care Welfare Home received a drug delivery message from the Community Health Service Center on Linfen Road Street.

Many elderly residents in nursing homes are suffering from chronic diseases and need to take medication for a long time. Traditional prescriptions require family members to go to the hospital for registration, payment, and medication collection at least every two weeks. "We have a medication list for every elderly person, which is updated every day. Previously, the first thing I do at work is to check the medication list and see which elderly person's medication is about to run out. I have to call their family to remind them to dispense medication for the elderly person." Zhao Bin said, the biggest concern is that their family members are too busy or "careless" and forget to dispense medication. "The elderly person's chronic disease medication cannot be stopped at once.".

In the "five bed linkage" mode, the daily medication problem of the elderly has been solved. The Community Health Service Center of Linfen Road Subdistrict has set up an account for each contracted elderly person at the Elderly Care Welfare Home. With the help of nursing staff, family doctors in the community can provide a series of remote medical services for the elderly through the "Health Quiet" platform on their mobile phones, such as video face-to-face consultations, chronic disease follow-up, and traditional Chinese medicine body mass identification. Family doctors can complete diagnosis and treatment records, laboratory tests, medication dispensing, accounting, and delivery online through the "Jing'an Family Doctor Home Bed Mobile App". "There is no need for family members to go to the hospital anymore. Community family doctors can prescribe chronic disease medication for the elderly, which basically meets their medication needs. The elderly can also swipe their medical insurance card to pay part of their medication fees."

Elderly people at home can also enjoy the "Five Bed Linkage" service

Under the "Five Bed Linkage" model explored in Jing'an District, elderly people who have signed up for "Family Elderly Care Beds" will also be able to enjoy these services.

"Family Elderly Care Beds" is a home-based elderly care model that Shanghai is exploring, which extends the care services of elderly care institutions to the home.

Ye Xiuyi, who lives in Shimen Second Road Street, is 92 years old this year. Her spouse is the same age as her, and the two are enjoying the "family nursing bed" service. The two elderly people are from a pure elderly family, and their children are not around for a long time. The two elderly people are physically strong and can basically take care of themselves. For this purpose, the "Family Elderly Care Beds" consultant has tailored a set of basic elderly care services based on their physical condition and personal needs. For example, emergency call "one click communication" devices are equipped for elderly families to receive timely assistance in case of accidents; Both elderly people have chronic diseases and it is inconvenient to go to the community health service center for treatment. Therefore, nursing staff were arranged to come to their homes twice a week to measure their blood pressure and blood sugar; Signed with a family doctor who can adjust medication for the elderly based on feedback from caregivers.

But in an accident, Grandma Ye accidentally fell and broke a bone. Her spouse is unable to take care of her. In the "Five Bed Linkage" mode, the "Family Elderly Care Bed" consultant arranged for her to rest at the community elderly care home, with professional caregivers taking care of her daily life. After her basic physical recovery, she returned to her family and supported each other with her spouse, enjoying the rest of her life.

The reporter learned that the "five bed model" has been piloted for a period of time between elderly care institutions and medical institutions such as the Elderly Care Welfare Home, Linfen Road Street Community Health Service Center, and Shibei Central Hospital, and has been welcomed by the elderly and their families. From August this year, Jing'an District will further promote this model throughout the district.
