Third class doctor: Nearly a thousand people have been treated in a year and a half, and have these symptoms. Adult ADHD? 45 year old man and child diagnosed together in outpatient clinic | adult | symptoms
In Zhejiang, there is only one "adult attention deficit/hyperactivity disorder" clinic. in run run run hospital affiliated to medical college of Zhejiang university, there are only 15 clinics every Wednesday afternoon. On July 20, our reporter searched the registration applet and found that the clinic had been fully booked within three weeks, and the earliest appointment could only be made in 27 days.
In the outpatient doctor Qin Yanhua's reception records, many adults had been transferred to the learning difficulties, children's psychology department and other medical treatment, and then referred.
What kind of clinic is this? What are the symptoms of "ADHD" group? Our reporter visited and found that, compared with the hidden huge patient population, the number of diagnosed patients may be the tip of the iceberg.
Many adults have moved from pediatrics.
Nearly 1,000 people were admitted in a year and a half, and about 20% were diagnosed.
On the afternoon of July 19, Wang Cheng, a 28-year-old boy, was diagnosed with "adult ADHD" after consultation and examination in the "adult ADHD" clinic in area D, 5th floor of run run run hospital of Zhejiang University ".
"After finishing an EEG with a cognitive task, the doctor looked at the report and said that my brain is different from ordinary people." Wang Cheng needs to cross out 3 from the numbers on the whole screen of the computer, and when he is distracted, he will cross it to 8, or miss it, which is very difficult for people who are distracted.
From childhood to adulthood, teachers and parents commented on Wang Cheng as "deserted in class", "lost in class" and "like to interrupt in class". Wang Cheng's mother had always thought that "her son's brain is weak", but she didn't know it was also a kind of disease. "Wang Cheng wears shoes to school and won't notice that the two shoes are different, and she will lose the school uniform." At ordinary times, his family asked Wang Cheng to throw away the garbage by the way. He did not remember it once, and then everyone stopped telling him.
Shen Wei, 45, is currently the oldest patient diagnosed with Qin Yanhua. Shen Wei's two children were diagnosed with "child ADHD" in the Children's Hospital Affiliated to Zhejiang University. He found that he was quite consistent with the control symptoms, so he went to the "adult ADHD" clinic recommend the pediatrician.
"The heritability of ADHD is 70% to 80%. Many adults realize that they may also have ADHD because their children have been diagnosed with ADHD." According to Qin Yanhua, patients can actually detect abnormalities in their lives, but few people understand "adult ADHD", let alone where to see a doctor.
Up to now, the "adult ADHD" clinic of run run run hospital of Zhejiang University has been opened for less than a year and a half. Qin Yanhua has received nearly 1000 people, of which about 20% have been diagnosed. Data show that the age of patients ranges from 20 to 45 years old, and people around 30 years old account for the majority.
Many patients describe themselves like this, "procrastination", "inattention, loss of attention", "often interrupt other people's conversations, and answer before the question is finished"......
Since no suitable clinic has been found before, many patients have gone to the learning difficulty clinic and children's psychological and behavioral clinic associated with pediatric ADHD with the psychology of giving it a try.
Diagnosis is a complex process.
There are different methods in foreign countries, and there are no medical guidelines in China at present.
A number of parents of "adult ADHD" patients at the scene reported that looking back on their child's growth experience, the family had to "put 120 a mind's eye" on him, because the child would do a lot of unreliable things, "talk to him, he listened in his left ear, and his right ear went out." "forget the cell phone, forget the key, forget the access card, often forget things......"
One female patient described, "My attention is distracted, like myopia, I can't focus, and my vision is blurred. The treatment is like giving me a pair of glasses, which allows me to focus on specific things."
However, Shen Jie, a follow-up patient with "adult ADHD", felt that although he had improved a lot after taking the medicine for more than a year, he still could not complete the process of arranging the meeting schedule. "For me, no matter how much medicine I take, this is an impossible thing to accomplish."
Shen Jie's decision to return to China for medical treatment stems from a failed job search experience after he graduated from studying in Japan. During the whole interview, Shen Jie wanted to escape every moment. "At that time, everyone was sitting in a suit. I felt very restless. After a minute, I felt like half an hour. At that time, I felt different from others."
"Adult ADHD" is called "unreadable air" in Japan, and it can be understood as "low EQ" in China ". When to speak and when not to interrupt is difficult for "adult ADHD" patients to distinguish.
"Mental diseases, including'adult ADHD, 'have to go through a complex and multiple assessment, and it is clear that they do not conform to other diseases. Only after exclusion can they be considered as mental diseases." Qin Yanhua told reporters that the history of "adult ADHD" should be at least started from an early age. Diagnosis is a complex process and a challenge for all mental illnesses.
"At present, there are no medical guidelines for adult ADHD in China, and there are no official recommend medical guidelines in the United States. The medical guidelines in the United Kingdom and Germany will put drugs in the front position of comparison." Qin Yanhua said that at present, the treatment method for "adult ADHD" is relatively simple. She will give some drug treatment to patients whose lives are greatly affected, combined with behavioral treatment and psychotherapy. If there is little impact on the life of patients, will prefer to do behavioral therapy.
A neglected "alternative group"
Zhejiang or 1.18 million "adult ADHD" patients
Compared with the well-known "child ADHD", "adult ADHD" is not widely recognized.
In March 2022, when the Shaw Hospital of Zhejiang University opened the "Adult ADHD" clinic, only two numbers were listed in the first month, and one of the patients was an employee of the hospital. However, the outpatient service has been opened for less than half a year, and the appointment registration will have to be arranged 20 days later. At present, there is only this "adult ADHD" clinic in Zhejiang, and such clinics are rare in the country.
According to the World Health Organization's Global Epidemiology Survey, the average prevalence of "adult ADHD" is 2.8 per cent, with 3.6 per cent in high-income groups, 3.0 per cent in middle-income groups and 1.4 per cent in low-income groups. According to relevant data, "adult ADHD" affects at least 25.2 million people in my country, and there may be 1.18 million "adult ADHD" patients in Zhejiang Province.
Chen Wei, a doctoral supervisor at Zhejiang University and director of the Department of Mental Health at Run Run Run Shaw Hospital of Zhejiang University, told reporters that the number of confirmed patients is less than a fraction of the number of hidden patients. This is a group of groups that need to be paid attention to by themselves and society.
Some studies believe that "adult ADHD" is related to genetics and environment, but the disease is still a medical problem, and the cause and treatment are not clear.
