On August 29, Beijing, Xinhua News Agency reported that at around 16:00 on November 21, 2022, a particularly serious fire accident occurred at Anyang Kaixinda Trading Co., Ltd. in Wenfeng District, Anyang City, Henan Province, resulting in 42 deaths and 2 injuries, and a direct economic loss of 12.311 million yuan. Recently, the Executive Meeting of the State Council approved the investigation report on the "11.21" special major fire accident of Kaixinda Trading Co., Ltd. in Anyang, Henan Province. After investigation by the accident investigation team of the State Council, it has been determined that the "11.21" special major fire accident of Kaixinda Trading Co., Ltd. in Anyang City, Henan Province was a production safety responsibility accident caused by the serious violation of laws and regulations by the enterprise leader, failure to fulfill the main responsibility, and inadequate performance of local party committees, governments, relevant departments, and units.
After the accident, the Central Committee of the Communist Party of China and the State Council attached great importance to it.It is emphasized that all regions and relevant departments should always adhere to the principle of putting people first and life first, strengthen their responsibility for safety production, comprehensively investigate and rectify various risks and hidden dangers, and resolutely prevent and curb the occurrence of major accidents.The Henan Provincial Party Committee and Government, as well as the Anyang Municipal Party Committee and Government, immediately organized emergency rescue and post accident disposal work after the accident occurred.
According to relevant laws and regulations, and with the approval of the State Council, a special investigation team for the "11.21" major fire accident of Kaixinda Trading Co., Ltd. in Anyang City, Henan Province was established, led by the Ministry of Emergency Management, with the participation of relevant officials from the Ministry of Public Security, the National Fire Rescue Bureau, the All China Federation of Trade Unions, and the People's Government of Henan Province. Experts were hired to participate in the accident investigation. The accident investigation team investigated the accident process, causes, casualties, direct economic losses, and relevant units through on-site inspections, testing and identification, video analysis, simulation experiments, and data retrieval, questioning and discussions. They identified the problems and responsibilities of local party committees, governments, relevant departments, and units in terms of supervision, and proposed suggestions for rectification and prevention measures.
The accident investigation team found out that the direct cause of the accident was the illegal and illegal electric welding operation of the person in charge of Cassida in the warehouse on the first floor. The high temperature welding slag ignited the packaging carton, and the bottled polyurethane foam caulking agent in the carton exploded and ignited when heated, thus causing a large amount of butter, self spray paint, rust remover, bottled butane and gloves for cassettes, rubber products, etc. to burn and spread rapidly one after another, and producing a large amount of high-temperature toxic smoke. When a fire broke out, some of the fire-fighting facilities in the first floor warehouse of Kaixinda Company were missing, and the second floor was artificially shut down and failed. The responsible person of Shangxin Company did not organize the evacuation of employees in a timely and effective manner, which was an important cause of a large number of employee casualties.
The main problems exposed by the investigation and investigation of the accident are the illegal and irregular organization of construction by the involved enterprises, violation of fire safety regulations, etc. The local party committees and governments have not conscientiously implemented their local safety management responsibilities, the supervision and inspection of fire rescue institutions and special rectification are inadequate, the commercial departments have not provided sufficient guidance and supervision on fire safety management in the commercial industry, the daily fire supervision and inspection of public security police stations are not in place, the emergency management departments have not been strict in identifying safety hazards in production, the natural resources departments have not thoroughly investigated and dealt with illegal land use behavior, and the housing and urban-rural construction and urban management departments have not investigated and dealt with fire safety issues related to construction projects.
The accident investigation team shall, in accordance with regulations, transfer the clues and relevant materials found during the investigation to the Central Commission for Discipline Inspection and the National Supervisory Commission for accountability review and investigation team regarding the problems in the performance of local party committees, governments, and relevant department public officials.
