Supply and demand cannot match, and training for caregivers is urgent

By the end of 2022, the population aged 60 and above with registered residence in Shanghai will account for 36.8% of the total population, reaching 5.5366 million, of which more and more elderly people can not take care of themselves and "empty nest elderly". Li Jin, Vice Dean of Renji Hospital Affiliated to Shanghai Jiao Tong University School of Medicine, admitted that the increasing diversity of services and the requirement for unaccompanied wards in medical institutions have also brought unprecedented challenges to medical nurses. "Insufficient numbers, low professional knowledge and skills have become the biggest problems restricting the development of caregivers."
Cheng Huafeng, former Deputy Secretary of the Party Committee of Renji Hospital, introduced that the South Campus team of Renji Hospital had conducted research on nursing staff in several medical institutions in Pujiang Town, Minhang District. The results showed that the nursing staff had low educational qualifications, with less than half having a junior high school or higher education, about 30% being illiterate, and almost no nursing theoretical foundation; The vast majority of caregivers are over 50 years old, and there are very few under 45 years old. In 2019, the National Health Commission and five other departments jointly issued the Notice on Strengthening the Training and Standardized Management of Medical Nurses, setting standards at the national level and promoting the professionalization and standardization of medical caregivers. In 2020, Shanghai issued relevant work requirements based on actual conditions, clarifying the training and management of caregivers. In July of the same year, the Minhang District Health Commission took the lead in collaborating with the South Campus of Renji Hospital to pilot standardized training for elderly care workers. The first batch of training bases for elderly care workers were awarded to six medical institutions. This model, which was jointly developed by local governments and medical institutions, effectively explored the professional development of elderly care workers and provided decision-making basis for the standardized training and management of subsequent medical care workers.
Training is inseparable from standardized and homogeneous teachers, teaching materials and suitable venues. In May 2021, Minhang District was approved by the Shanghai Nursing Association to set up a medical nurse training assessment point. In September 2022, the first institution in the district obtained the medical nurse training qualification.
Breaking the problem of "proving for evidence" and exploring standardized training

In fact, as early as July 2020, the South Campus of Renji Hospital took the lead in collaborating with Rongheng Vocational Skills Training Co., Ltd. to establish the "Minhang District Elderly Nursing Staff Standardization Training Guidance Expert Group", formulated a standardized training implementation plan, including writing training tutorials, forming a joint teaching team, producing relevant online teaching courseware, and innovatively proposed "graded use". From August to December of the same year, the expert group piloted free training for nursing staff. 388 people obtained the Level 5 certificate qualification, 133 people obtained the Level 4 certificate qualification, and some students achieved good results by winning first place in the "First Shanghai Municipal Medical Institution Nursing Professional Skills Competition Finals" organized by the Shanghai Medical Union. "Afterwards, we launched a pilot program for medical nurse grading services in five departments of the South Campus of Renji Hospital. 80% of patients expressed that exploring nurse grading is beneficial for patient recovery, and the average score of monthly assessments has also been improved. The on duty rate, stability, and professional identity have also significantly improved," Li Jin said.
